The Personal Intake Form is a convenient way to input your information into your Mediation case file. The information requested here is important for your case file and ultimately your court documents and agreements.
To input your Intake Data:
1-Check your email for an invitation to complete the Intake Form
2-When you click on the link in the email you’ll be taken to the online form.
3-Input your data and submit it.
A few Notes about the information you enter:
-Please enter everything you can. If you don’t enter in some of the information I will have to follow up with you later to request it, which can add some time and cost to your fees.
-Use Proper Capitalization. The data you enter will be what is used on the Court Forms and Documents later in the process. If you make sure proper spelling and capitalization are used at this stage we can avoid mistakes and time later correcting mistakes.
-If you don’t have all the data ready you can pause the data entry. If you need to stop and look up a SSN or something you can pause the data entry and simply close the web page. When you are ready click on the original link and you can resume your data entry without having to retype your data again.
-Date and Place of Marriage. This should be what is on the Marriage Certificate and not reference the ceremony date and place.
-If you are unemployed, or a Home Maker then under Employer Name just type in Unemployed, or Home Maker and leave the address empty.
-If you don’t have minor children, please leave those fields empty. No need to type in “N/A” or anything.
-Form Fillers. Most Form filling plugins do a fair job but please review the fields after forms are auto filled. Sometimes the person’s is input under child’s name for example so just do a quick review to clean up any errors.
