Completing the Payment Intake Form

Your Method of Payment is entered into the Payment Intake Form. This is required to be on file before we can schedule mediations and work together.

1-Check your email for an invitation to enter your Payment Intake Form.

2-Click on the link in the email and you’ll be taken to your payment intake form.

3-Enter in a valid credit or debt card that will be used for Mediation Fees.

4-Submit the form
